The most important part of your care system's participation in the MNCARES study of care coordination is about to begin with submission of data for patients receiving care coordination to MNCM. We are asking clinics to identify all patients aged ≥18 years who meet criteria for each of the following two groups:

  • Historical Cohort: Began receiving care coordination services at a participating clinic between 01/01/2018 and 02/28/2019
  • Primary Cohort: Began receiving care coordination services at a participating clinic between 01/01/2021 and 12/31/2021

Participating clinics will be asked to submit data associated with patients in the Historical Cohort in the summer of 2021 (mid-June through mid-August) and for patients in the Primary Cohort in spring-summer of 2022.

With the questions below, we are requesting information about your clinic's/care system's ability to identify patients meeting criteria for the Historical Cohort, as described above. Specifically, we seek to understand any challenges your clinic(s) may have in identifying this group of patients in your data system(s) for historical patient records.
